For people with disabilities, our Individualised Living Service supports you to live in a home you choose, in a way that suits you.
We work side-by-side with you and your network to explore what your ideal living arrangement might look like.
You’ll also get support to:
Choose where you live and who you live with.
Explore and connect with the daily support you need.
Build relationships and get involved in the community.
Ensure the living arrangement continues to work for you through regular check-ins.
At Uniting Communities, our Individualised Living Service provides you with the opportunity to have a housemate live in your home, or you can live with a host in their home. At Uniting Communities the host or housemate are contractors. We call these people an ‘Independent Living Coach'.
We will guide you through the process of selecting the right Independent Living Coach for you. Together, we’ll make sure you find a match who suits your needs, goals, values, and lifestyle.
Your Coach will support you with day-to-day activities, and work alongside you to provide coaching so that you can develop skills, make connections, and grow your confidence.
With your Coach by your side, you’ll focus on achieving your NDIS goals, building new skills and preparing to live independently in the future.
Who we support
The Individualised Living Service supports NDIS participants aged 18 and over who:
Have an intellectual, psychosocial, physical disability or acquired brain injury.
Have NDIS Exploration and Design funding.
We’re here to ensure you feel heard, understood, and supported to express your goals and needs. We also know how important relationships are for your wellbeing, so we’ll help you build and strengthen connections with friends, family, and the community.
What you can expect
Uniting Communities’ will support you in creating a life you choose and build independent living skills.
With the Individualised Living Service you will:
Be encouraged to live a healthy and happy lifestyle of your choice.
Have a Coach who lives with you (in your home or theirs) to provide ongoing coaching and support.
Work with your Coach and the ILS team to create opportunities to achieve your life goals.
Engage with local community activities and services.
Continue your regular activities, like going to work or doing daily activities, seeing your friends or family, participating in your hobbies and accessing your existing support workers.
